This role is responsible for building and maintaining relationships with hospital staff, healthcare providers, and community organizations to streamline referral processes. Day-to-day tasks include coordinating patient referrals, serving as a bridge between hospitals and the foundation's services, managing communications, and identifying opportunities to improve processes for patient care and outcomes. The Liaison will also ensure compliance with regulatory requirements and contribute to the foundation’s mission of providing exceptional healthcare support.QualificationsRelationship-building and networking skills to establish and maintain strong partnerships with hospitals, healthcare providers and non-profits.Strong organizational and communication skills to manage patient referrals, coordinate with multiple stakeholders, and provide clear updates and documentation.Knowledge of healthcare systems, referral processes, and applicable regulations or compliance standards.Problem-solving and analytical skills to identify and address challenges in the referral and care coordination process.Proficiency in using relevant software or databases for managing patient information and referrals.Flexibility to work on-site in Greater Philadelphia and travel locally as needed.Prior experience in healthcare, social services, or related fields is highly desirable.A bachelor’s degree in Healthcare Administration, Social Work, or a related field is preferred. An associate's degree is accepted if it's the right fit.
