The Field Clinical Performance Manager oversees the clinical competency, performance, and professional development of home health field nurses. This leadership role ensures high-quality patient care delivery, regulatory compliance, and operational excellence through recruitment, onboarding, training, mentorship, and ongoing performance management of clinical staff.
The position combines clinical expertise, workforce development, and quality improvement to elevate standards of care across the organization’s home-based services.
Key Responsibilities
Clinical Workforce Recruitment & Onboarding
Partner with Human Resources to interview and select qualified RNs and LPNs/LVNs.
Evaluate clinical skills, home health knowledge, and cultural alignment.
Lead structured onboarding and orientation programs for new field clinicians.
Validate clinical competencies before assigning patients independently.
Clinical Education & Training
Design, implement, and evaluate comprehensive training programs for field nurses.
Conduct competency-based education on:
Skilled nursing procedures
Wound care management
Medication administration and reconciliation
Chronic disease management (CHF, COPD, Diabetes, etc.)
Infection prevention and control
OASIS documentation and care planning
Facilitate case reviews, skills labs, and clinical simulations.
Provide remediation training when performance gaps are identified.
Field Supervision & Performance Management
Conduct supervisory visits and field observations in patient homes.
Provide real-time coaching, mentoring, and professional guidance.
Monitor productivity, documentation accuracy, and adherence to care standards.
Develop and implement performance improvement plans as needed.
Serve as a clinical escalation point for complex patient cases.
Quality Assurance & Regulatory Compliance
Ensure compliance with state and federal home health regulations, including Medicare Conditions of Participation.
Support Quality Assurance and Performance Improvement (QAPI) initiatives.
Audit clinical documentation for accuracy, timeliness, and regulatory compliance.
Identify trends, risks, and educational opportunities to improve outcomes.
Stay current on evolving home health regulations and best practices.
Professional Development & Retention
Foster a culture of clinical excellence and continuous improvement.
Promote leadership development among field staff.
Coordinate continuing education opportunities.
Support employee engagement and retention initiatives.
Qualifications
Active Registered Nurse (RN) license in good standing.
Bachelor of Science in Nursing (BSN) required; Master’s preferred.
Minimum 3–5 years of home health clinical experience.
Prior experience in clinical supervision, education, or leadership preferred.
Strong working knowledge of OASIS and home health regulatory standards.
Demonstrated ability to mentor and evaluate clinical staff.
Excellent organizational, communication, and leadership skills.
Ability to travel locally for field supervision.
