Must have strong Hospice Experience. Must have HCHB After worker clears onboarding, Online learning is required. After the online learning is complete, 8 hour in person learning is required and schedules as "first day". Once this is done shifts may be assigned to the worker. *8 hour in person class is held at the Knowledge Center: West Palm Beach FL 33407 *Cannot do in person learning until online is complete. If not complete in time worker is pushed to the next month's start and learning schedule. Orientation is scheduled for "once a month"Duties:Job Summary: Our client is seeking a Home Health LPN - Licensed Practical Nurse to provide health care services in accordance with prescribed medical treatment and agency policy in the home according to the patient's plan of care. May perform certain nursing procedures under the supervision of a Registered Nurse. -Schedule: M-F 40 hours plus weekend rotation; Full-time.Job Responsibilities: -Provide high quality, patient-centered care in the home setting. -Build meaningful relationships with patients and their families. -Provide nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient's plan of care under the direction and supervision of a Registered Nurse. -Observe patients and report adverse reactions to medication or treatment to the Clinical Coordinator. -Assist the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures. -Perform patient and family instruction on diet, medication, and treatment.Skills:Required Skills & Experience:-Minimum of one (1) year of current applicable experience.Preferred Skills & Experience:-N/AEducation:Required Education:-Graduate of an approved nursing program.Preferred Education:-N/ARequired Certifications & Licensure:-Current Florida LPN License.-Current BLS.Preferred Certifications & Licensure:
