Long-Term Care Clinical PharmacistOVERVIEW:Infinium Pharmacy is a privately owned and independently operated, Long Term Care Pharmacy, dedicated to meeting the growing need for personalized pharmaceutical care in Skilled and Assisted Living facilities.PURPOSE OF POSITION:The Clinical Pharmacist is essential to ensure accurate and complete service for patients. This position reviews profiles for correct drug, dosage, and route of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed. This position may also require medication therapy management training and implementation as well as monthly medication reviews for the facilities, as required. The Clinical Pharmacist may fulfill the role of a Consultant Pharmacist.SUMMARY OF DUTIES:Reviews prescription orders issued by physicians and other authorized prescribersPerforms medication profile reviewsExamines prepared orders for accuracyProvides clinical drug information to meet the needs of the patient, physician, or other healthcare professionalCoordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical suppliesProvide quality on-call pharmacist services during closed hours of pharmacyMaintains established procedures concerning medication packaging standards, quality assurance, and security of controlled substancesEnsures compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required recordsAssist with any duties necessary to ensure the functionality of the businessParticipates in consulting pharmacist dutiesMonthly Medication reviews for residents of nursing facilities, either on-site or off-site, as deemed appropriate.Medication room inspections at nursing facilities as required by regulation or by contract.Medication Pass Observations at nursing facilities as required by contract.Attendance at Facility Meetings (as contracted) and presentation of relevant clinical information.Other duties as assigned.SUPERVISORY DUTIES:This position has no supervisory responsibilities.KNOWLEDGE, SKILLS, and ABILITIES:EducationMinimum of a Bachelor’s degree from an accredited college or university required with a degree in pharmacy.Missouri Pharmacist License (must be in good standing)QualificationsStrong data entry/computer processing skillsStrong organizational and time management skillsGreat attention to detail, accuracy, and thoroughnessAbility to work in a fast-paced environmentCompliance to all HIPAA/privacy regulationsCompetenciesOral Communication--Speaks clearly and persuasively in positive or negative situations; Listens and gets clarification; Responds well to questions; Demonstrates group presentation skills; Participates in meetings.Written Communication--Writes clearly and informatively; Edits work for spelling and grammar; Varies writing style to meet needs; Presents numerical data effectively; Able to read and interpret written information.Dependability--Follows instructions, responds to management direction; Takes responsibility for own actions; Keeps commitments; Commits to long hours of work when necessary to reach goals; Completes tasks on time or notifies appropriate person with an alternate plan.Work Environment/Physical DemandsContinually utilize visual acuity to read fine and/or small print, and to work off a computer screen for up to 10 hours/dayFrequently sitting and/or standing for up to 10 hours/dayHaving a range of motion from kneeling, to bending over to raising arms above head to frequently retrieve stockFrequently required to lift and/or carry up to 40 poundsTravel: 0%-25%A background check and pre-employment drug screen is required.Infinium Pharmacy is an equal opportunity employer.
