Family Practice - Geriatrics Physician

Schedule • Flexible scheduling based on provider availability• Open to weekdays, evenings, and weekendsThis position involves completing in-home health assessments for a local health plan in Fulton County, GA. The schedule is flexible, with weekday, evening, and weekend options. At-home assessment experience is preferred but not required.$100 per completed assessment,$50 for no-shows$70 health, if patient does not want provider to come inside home , rare occasionsmileage reimbursement after 30 miles. ResponsibilitiesConduct comprehensive histories and physical exams, evaluate social determinants, and deliver health education.Order diagnostics, develop and adjust treatment plans, and manage medications. (as applicable)Educate on disease prevention, medication use, lifestyle changes, and self-care Complete and submit visit documentation within 24 hours (this should be completed during the visit and shouldn't have much to do at home). Strong documentation and time management skills Required SkillsNP/PA LicenseAt least 1 year of experience Ability to commit to 30 assessments within 1 month
