About the Role As a Field LPN, you will provide skilled, patient-centered care in the home setting, following individualized Plans of Care under the supervision of a Registered Nurse. Your work will help patients remain safe, comfortable, and independent in their homes.Experience with WellSky/Kinnser and home health documentation is a plus! Key Responsibilities Provide safe, effective nursing care per the individualized Plan of CareCommunicate promptly with the clinical team regarding changes in patient condition or emergenciesPerform skilled nursing services, including:Medication administrationWound careInfusion therapyCatheter careSafe transferring and positioning of patientsCollaborate with the RN Supervisor and interdisciplinary care teamDemonstrate empathy, professionalism, and attention to detail at every visit Requirements & Qualifications Active Florida LPN licenseWound care experience requiredIV certification preferredMinimum 1 year of home health experience preferredStrong communication, teamwork, and customer service skillsProficiency with electronic medical records (EMR), preferably WellSky/Kinnser Why Join Activa Home Health?
