We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients, assisting in their recovery and well-being. This role requires a strong commitment to patient care, effective communication skills, and the ability to work collaboratively within a multidisciplinary team. The ideal candidate will have experience in various healthcare settings, including home health, urgent care and infectious disease care. Your Impact as Licensed Practical Nurse As an LPN in our agency, you will: Provide direct patient care, including assistance with daily living activities, administering medications and treatments as prescribed by physicians Maintain accurate patient records and documentation in compliance with healthcare regulations. Conducts ongoing assessments to monitor health status and report changes. Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers. Observes client conditions and accurately reports to the registered nurse. Assists with client discharge or transfer. Performs all skills, treatments and procedures competently in accordance with the scope of Nursing Practice and adheres to the policies, procedures and protocols of the agency. Qualifications & Requirements Active LPN License in state of Oregon (Unrestricted) 1 year of nursing experience required; Community, home health or medical/surgical experience preferred. Current CPR certification. Valid driver’s license, reliable transportation and liability insurance. Negative TB test or chest x-ray.
