Occupational Therapist

Job Summary We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapy services to patients across various settings, including inpatient, outpatient, home health, schools, and skilled nursing facilities. This role involves developing personalized care plans, assisting patients with activities of daily living (ADLs), and supporting recovery and independence for individuals of all ages with diverse needs. The Occupational Therapist will collaborate with multidisciplinary teams to ensure optimal patient outcomes and quality of care. Duties Assess patients’ physical, cognitive, and emotional needs to develop individualized treatment plans. Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and mobility. Implement therapeutic interventions tailored to specific populations including pediatrics, geriatrics, individuals with autism, disabilities, or post-acute conditions. Conduct discharge planning and coordinate case management to ensure seamless transition across care settings. Work with patients in various environments such as hospitals (acute care, ICU), outpatient clinics, schools, skilled nursing facilities, and home health. Collaborate with physicians, speech therapists, physical therapists, and other healthcare professionals to optimize patient care. Educate patients and families on therapy techniques, adaptive equipment use, and strategies for maintaining progress at home or in community settings. Maintain accurate documentation of patient progress and treatment outcomes in compliance with healthcare regulations. Participate in team meetings and contribute to the development of comprehensive care plans that address medical terminology, physiology, anatomy, and patient-specific goals. Skills Strong knowledge of anatomy, physiology, medical terminology, and patient care principles. Experience assisting with ADLs and discharge planning across diverse populations including pediatrics (early intervention), geriatrics, and individuals with disabilities or autism. Skilled in case management and developing effective care plans tailored to individual needs. Proficiency working in various settings such as inpatient units (including ICU), outpatient clinics, skilled nursing facilities, school environments (special education), and home health services. Excellent communication skills for working with patients of all ages as well as interdisciplinary team members. Ability to adapt therapy techniques for different populations including post-acute care patients and those requiring occupational health services. Knowledge of speech therapy integration is a plus for comprehensive patient support. Experience working within acute care environments or skilled nursing facilities is preferred.
