Makes follow up visits according to patient, family, and facility needs.Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.What is Required?Current, unrestricted state RN licensee1+ year experience in a clinical care setting as a nurseAble and willing to travel within branch/office coverage area.Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentMust have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.Must be able and willing to travel 50%Associates Degree or Bachelor's Degree in Nursing is preferredPrevious Home Health or Hospice Experience is preferredPrevious experience with HomeCare HomeBase is preferredYou will report to the Clinical Manager or Clinical Supervisor.#LI-KZ1Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
