Role Description This is a full-time, on-site/on-the-road role located in Philadelphia, PA, 19131 for a Case Manager. The Case Manager will oversee client caseloads, provide individualized support services, develop tailored care plans, and monitor progress to ensure client needs are met. The role includes coordinating with families, community agencies, and service providers, while maintaining accurate documentation and ensuring compliance with organizational policies and relevant regulations. Qualifications Understanding of case management principles, client support services, and care plan developmentStrong communication and interpersonal skills for family and community engagementProficiency in documentation, record-keeping, and maintaining confidentialityKnowledge of social work practices and a commitment to client welfareAbility to work collaboratively with diverse populations and address sensitive situations professionallyFamiliarity with applicable federal, state, and local regulationsProficiency with scheduling, time management, and prioritization of tasksBachelor's or Master's degree in Social Work, Human Services, or related field; relevant experience is a plus
