What You'll Do As an Assistant Care Manager for the Olympia area, you'll help ensure clients receive quality care and support while partnering with caregivers, families, case managers, and healthcare professionals. Responsibilities Include:Coordinate and complete new client assessmentsDevelop and update client care plans and task listsPartner with Medicaid, VA, ODDS, OPI, and other community case managersConduct ongoing client visits and quarterly assessmentsSupport client scheduling and coordination needsMonitor quality of care and client satisfactionIdentify client safety, mobility, hygiene, and support needsHelp create safe and successful home care plansProvide excellent customer service to clients, families, and caregiversParticipate in an on-call rotation for support and staffing coordinationMaintain accurate documentation and confidentiality What We're Looking For We're looking for someone who is compassionate, dependable, organized, and able to balance multiple priorities while staying calm under pressure.
