This fully remote, flexible role is ideal for a self-motivated provider who excels in patient engagement, clinical assessment, and proactive care planning within a telehealth environment.Location: 100% RemoteSchedule: Flexible hours – set your own scheduleEligibility: Must be a Hawaii resident with an active Hawaii NP or PA-C licenseJob SummaryWe are seeking a dedicated and compassionate NP or PA-C to conduct virtual AWVs and ACP sessions for Medicare patients. You will conduct comprehensive assessments, facilitate structured ACP discussions, and identify and close care gaps. You will collaborate with interdisciplinary teams and maintain accurate documentation.Key ResponsibilitiesConduct comprehensive AWVs for Medicare beneficiaries via secure telehealth platforms.Facilitate structured ACP discussions, documenting patients' goals and care preferences.Review medical histories, medications, functional and cognitive status, and preventive care needs.Identify and close care gaps by guiding patients on screenings, vaccinations, and chronic disease management.Collaborate with interdisciplinary care teams and primary care providers to ensure coordinated, patient-centered care.Maintain timely, accurate documentation in the EHR.Adhere to CMS guidelines and clinical quality standards.QualificationsActive, unrestricted Hawaii NP or PA-C license.Minimum 3 years of clinical experience, including AWVs and ACP.Strong knowledge of Medicare guidelines and preventive care protocols.Proficiency with virtual care technology and EHR systems.Excellent communication and interpersonal skills.Bilingual in Tagalog or Ilocano is a plus (not required).Ability to work independently and manage a flexible virtual schedule.Preferred SkillsBackground in geriatrics, primary care, or internal medicine.Familiarity with HCC coding and risk adjustment.Experience in value-based care or accountable care organizations (ACOs).Why Join Us100% remote work with flexible scheduling.Opportunity to make a meaningful impact on preventive and personalized care for Medicare patients in Hawaii.Supportive and innovative clinical team environment.Competitive compensation structure.Seniority level: Mid-Senior LevelEmployment type: Part-timeJob function: Health Care Provider – Hospitals and Health CareJ-18808-Ljbffr
