Care Management Platform Consultant
Job Summary We are seeking a Care Management Platform Consultant to support a health plan client through the final phases of a care management platform implementation. This role will focus on workflow design, system configuration, testing support, and go-live readiness. The ideal candidate has experience working with care management platforms such as Helios, GuidingCare, Jiva, CareProminence, CareAdvance, or similar systems and can translate business and clinical workflows into system functionality. ResponsibilitiesSupport workflow design, configuration, and validation for a care management platform implementationTranslate operational and clinical requirements into system behavior and configurationConfigure and validate rules, triggers, member identification logic, cohorts, workflows, and related functionalityPartner with client teams during testing to troubleshoot issues and confirm expected outcomesSupport UAT, defect triage, and go-live readiness activitiesFacilitate discussions with clinical, operational, and technical stakeholdersIdentify gaps between requirements, workflow design, and system configurationProvide practical recommendations to improve workflow efficiency and platform usabilitySupport implementation of new platform functionality as it is releasedRequired Qualifications5+ years of experience supporting care management, utilization management, population health, or health plan operationsHands-on experience with at least one care management platform, such as:Helios / VirtualHealthGuidingCareJiva / ZeOmegaCareProminenceCareAdvance / MHKSimilar care management or population health platformsExperience translating workflows into system configurationStrong understanding of rules-based workflows, triggers, member identification, cohorting, or care management automationExperience supporting testing, UAT, issue resolution, or implementation readinessStrong client-facing communication skillsPreferred QualificationsPrior Helios / VirtualHealth experienceClinical background such as RN, LCSW, or similarExperience supporting Medicare, Medicaid, or commercial health plan populationsPrior experience with late-stage implementations, go-live support, or system stabilizationAbility to work across business, clinical, and technical teamsAbout Impresiv Health: Impresiv Health is a healthcare consulting partner specializing in clinical & operations management, enterprise project management, professional services, and software consulting services.
