This individual should be familiar with Targeted Case Management and be highly organized to complete assessments, care plans, and monitor the patient's needs. Flexible work schedule is required for this position to ensure continuity of care. Required Skills/EducationBachelors or Master’s Degree in human services, social work, or counseling to work with children 0-18 and their families in the home, community, and school. Needs to have experience working with systems, understand family dynamics, behavior management strategies, and work well with a treatment team.Strong organizational, liaison, and advocacy skills necessary.Prior experience in a mental health, human service, or school setting a plus.The Targeted Case Manager must hold a valid driver’s license and be able to demonstrate a clean driving and criminal record.
