This 13-week assignment is ideal for an experienced Home Health nurse comfortable managing autonomous field work, coordinating care across multiple counties, and building strong patient relationships in the community.ResponsibilitiesProvide skilled nursing care to patients in their homes in accordance with physician orders and plan of carePerform patient assessments, care coordination, and clinical documentationDeliver patient and family education regarding treatment plans, medications, and disease managementCollaborate with interdisciplinary care teams including therapy servicesManage daily visit schedules independently across assigned coverage areasSupport safe care transitions and continuity of careRequirementsActive Michigan RN license requiredBLS certification requiredMinimum 1 year of Home Health RN experience requiredStrong independent assessment and time management skillsReliable transportation for field-based patient visitsPrevious travel experience welcomed; first-time travelers acceptedAdditional Details13-week contract5x8 day shifts (8:00 AM–5:00 PM)40 guaranteed hours weekly5–6 patient visits per dayCoverage area includes Calhoun, Kalamazoo, and Van Buren countiesNo call requiredWeekend rotation approximately every 4th weekendFirst-time travelers acceptedLocals acceptedMinimum 1 year of Home Health experience required. Michigan RN license required. First-time travelers accepted. Must be available for full orientation week. Proof of flu vaccination required with submission.
