Family Home Provider
Position SummaryThe Family Home Provider (FHP) is responsible for providing a safe, stable, and supportive home environment for an individual with intellectual or developmental disabilities. The FHP delivers daily care, supervision, and skill-building supports consistent with the individual's Person-Centered Plan (PCP), while promoting independence, dignity, and community integration.RequirementsEssential Duties and ResponsibilitiesResidential SupportProvide 24/7 supervision and care in the provider's homeMaintain a clean, safe, and hazard-free living environmentEnsure adequate food, clothing, and personal care items are availableSupport daily living activities (e.g., bathing, grooming, toileting, meal preparation) Health and Medical SupportAdminister medications as prescribed (per Kentucky MAP guidelines if applicable)Schedule and transport to medical, dental, and therapy appointmentsMonitor health status and report concerns promptlyMaintain documentation of medications, appointments, and health-related incidents Person-Centered SupportImplement the individual's Person-Centered Plan (PCP) and/or Plan of CareSupport skill development in:Independent livingSocializationCommunicationBehavioral self-regulationEncourage choice-making and self-advocacy Community IntegrationFacilitate participation in community activities (church, recreation, employment, etc.)Provide transportation as neededPromote meaningful relationships and inclusion Behavioral SupportImplement approved behavior support plans (if applicable)Use positive behavior supports and de-escalation techniquesDocument and report incidents per agency and state requirements Documentation & ComplianceComplete required daily notes and service documentationMaintain compliance with:Kentucky Medicaid waiver regulations (SCL, MPW, or RISE)Agency policies and proceduresAdult/child abuse and neglect reporting lawsParticipate in audits, home inspections, and monitoring visits CollaborationCommunicate regularly with:Case managers/support brokersGuardians/family membersAgency staffAttend team meetings and required trainings
