Responsibilities/Job Description:Fairview is seeking a Nurse Practitioner to join our primary care practice at M Health Fairview Clinic River Falls. Join a collaborative and growing practice with a supportive, experienced care team dedicated to providing high-quality, patient-centered care.Practice Details:Established primary care practice with a collaborative team-based environmentPatient encounters average 18 patients per day1.0 FTE = 34 patient contact hours per weekOpenings: 0.8 to 1.0 FTE (Full Time) flexible scheduling optionsFully integrated Electronic Medical Record (EPIC)ResponsibilitiesAssess health status for specific patient populations using advanced practice knowledge and skills.Obtains relevant health and medical history.Performs a physical examination based on age and medical history.Performs/orders preventative and diagnostic procedures based on the patient's age and history.Identifies health and medical risk factors.Identifies acute and chronic health problems.Appropriately orders and accurately interprets diagnostic results.Develops and implements an individualized plan of care.Establishes practice agreements with collaborating physician.Collaborates with health care team members in developing realistic care expectations.Determines effectiveness of treatment plan with documentation of patient care outcomes.Revises and modifies the care plan as needed to achieve health and medical goals.Refers care to other health care professionals and community agencies as needed.Prescribes pharmacologic (including controlled substances) and non-pharmacologic agents.Communicates effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care.Provides education to patients, families or caregivers as needed.Documents according to established guidelines to facilitate ongoing quality care in a timely manner.Responds in a timely manner to phone calls regarding patient needs.Contributes to effective and efficient practice management policies and practices that support patient access to medical care.Participates in access planning for applicable area or population.Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures.Adheres to all relevant policies, procedures, and practice guidelines.Assists with the preparation for JCAHO and other regulatory surveys.Manages patient care through appropriate use of available resources.Appropriately prescribes and orders ancillary services.Utilizes Fairview referral network appropriately.Manages chronic health/disease conditions.Provides leadership for clinical and support staff.Serves as a resource to support staff, enhancing quality of care.Participates in the training, orientation, and evaluation of clinical and support staff.Participates in appropriate clinical investigation relevant to specific patient population.Responds to nursing staff educational needs in formal and informal venue demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities.Serves as consultant and role model for nursing staff.Maintains competence and professional development. Engages in continuing education.Is active in professional and other appropriate organizations.Demonstrates behaviors that support performance improvement activities.Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.Required QualificationsGraduate of an accredited school of nursing and formal nurse practitioner training program.BC/BE through the AANP or ANCCLicensed or ability to obtain APRN license with the Minnesota Board of NursingAbility to obtain and /or maintain DEA certification for the State of MinnesotaLicensed or ability to obtain Wisconsin RN licensure and Advanced Practice Nurse Prescriber (APNP) certification through the Wisconsin Board of Nursing.Ability to obtain and/or maintain DEA certification for the State of Wisconsin.Meet threshold criteria/qualifications for Credentialing and PrivilegesQualifications:$133,156 - $147,952 Annual *based on full time (1.0 FTE)
