Working in close collaboration with Registered Nurses and the interdisciplinary care team, LVNs apply sound clinical judgment to support curative, rehabilitative, palliative, and preventative care goals.This role requires a high degree of professional independence, clear and timely communication, and rigorous, compliant documentation. Within their defined scope of practice, LVNs make real-time clinical decisions that directly influence patient outcomes while building trusted, enduring relationships with clients and families. Care Indeed is committed to a culture that prioritizes teamwork, continuous professional development, and the overall well-being and success of every nurse on the care team.DUTIES & RESPONSIBILITIESDeliver skilled nursing care in home and community settings in partnership with the RN and interdisciplinary care teamUse clinical judgment to carry out the plan of care and support curative, rehabilitative, palliative, and preventative treatment goalsObserve, assess, document, and report patient status, including physical condition, mental status, symptoms, responses to treatment, safety concerns, and changes in condition using clear clinical terminologySupport patients in self-care, mobility, body mechanics, and rehabilitation activities aligned with the medical care planRecognize social, emotional, and environmental factors that influence patient well-being and provide care that promotes comfort, dignity, and safetyParticipate in care planning, team conferences, and ongoing learning that strengthen clinical skills and teamworkComplete accurate clinical and progress notes weekly, and as needed, using EMR systems such as WellSky through mobile or tablet platformsProvide clinical guidance and task delegation to Home Health Aides (HHAs) and Home Care Aides (HCAs) in alignment with the care plan and under RN oversightTREATMENTS & PROCEDURESLVNs may perform treatments and procedures allowed by the California scope of practice and Care Indeed clinical policy.Advanced LVN: An LVN with documented competency validation, additional training, and agency authorization to perform higher-skill procedures.Advanced LVNs may:Administer medicationsInitiate and manage peripheral IV therapy, including premixed medication and hydration fluidsFlush peripheral lines with saline or heparinInspect sites, change dressings, and remove peripheral IV cathetersCalculate and adjust IV flow ratesAssist the RN with blood checks and related proceduresJOB CONDITIONS & REQUIREMENTSActive California LVN license and current CPR certificationAbility to work independently in client homes and facilities and make real-time clinical decisions within the scope of practiceProficiency in spoken and written English for documentation and care coordination, with a commitment to communicating with clients in the language that supports their comfort and understanding whenever possibleExperience with electronic medical records, including systems such as WellSky, using mobile or tablet devices for timely, compliant documentationReliable transportation and the ability to travel between client locationsPhysical ability to lift up to 50 lbs and perform essential nursing duties in varied environmentsSuccessful completion of Live Scan fingerprint background clearance is required prior to employment.CPR PROVISION (Two-Week Window Option)A valid American Heart Association CPR certification is required. Candidates may be hired before obtaining it, but must secure certification within two weeks of hire and maintain it throughout employment. Proof must be submitted to the supervisor within the deadline.LICENSE/CERTIFICATION MAINTENANCERequired licensure/certification/registration must be maintained throughout employment. Proof of current, valid credentials must be provided upon request.WORK ENVIRONMENT & CARE SETTINGSCare is provided in diverse settings, including private homes, assisted or independent living communities, skilled nursing facilities, and hospitals, and involves close, direct interaction with clients. This role may include exposure to blood, body fluids, or other potentially infectious materials. It may require responding appropriately to urgent or emergencies in accordance with training and agency protocols.
