Registered Nurse (RN) | Home HealthLocation: Long Island City, NYAgency: Cynet HealthPay: $2,171 per weekShift Information: Days - 5 daysContract Duration: 260 WeeksStart Date: 7/2/2026About the PositionJob Title: RNProfession: NursingSpecialty: Home HealthDuration: 260 daysShift: 5x5 daysHours per Shift: Not specifiedExperience: Two (2) years of experience in a home setting within the last ten (10) yearsLicense: Current RN Licensure in ClientCertifications: BLS (to be obtained before start date)Must-Have:- All nurse assessors must have two (2) years of experience in a home setting within the last ten (10) years- Experience conducting comprehensive assessments of patients- Experience providing primary care to homebound individualsDescription:We are seeking a Registered Nurse for our home health specialty.The candidate must possess a current RN licensure in Client.A minimum of two years of experience in a home setting within the last ten years is required.Experience can include working in the field for certified home health agencies, conducting assessments, and providing care management.Candidates must have recent experience conducting assessments similar to the UAS criteria, such as OASIS.The ideal candidate will have experience caring for a geriatric population and managing chronic diseases in a home setting.Nurses will be responsible for performing initial in-home pre-evaluations of patients.They will evaluate health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans.Reviewing consumer medical documentation and health referral forms is also required.Evaluation data must be entered into electronic systems and transmitted as necessary.Candidates should document any concerns that surface during the evaluation process.Emphasis on continuity of care is essential to reduce fragmentation and gaps in treatment plans.Nurses will discuss health care options, needed supports, service vendor options, and waiver options with consumers.They will initiate communication between eligible consumers and managed care health plans for enrollment.Candidates should enhance communication and collaborative relationships with interdisciplinary care team members.Collecting quality review data and required documentation to support outcome measurements is part of the role.Identifying opportunities for health promotion and illness prevention is essential.Nurses must maintain a comprehensive working knowledge of community resources and payor requirements.They will meet all performance criteria and standards for the position as outlined in annual evaluations.Other duties may be assigned as necessary.RequirementsRequired for OnboardingBLSPharmacologySkills ChecklistSpecialty Exam
