COMMUNITY HEALTH EDUCATOR-GIMPOP

Key Duties & Responsibilities:
Will target individuals within the defined zip codes to determine their insurance status and wellness needs.
The goal is to help educate and navigate said individuals to get the below medical tests and appointments scheduled where needed: 1. Enroll at Cooper Green 2. Choose a primary care provider (PCP) (NOT an urgent care clinic nor emergency room) AND make an appointment to see them yearly 3. Get a Flu shot annually 4. For women age 40 or older, schedule a mammogram yearly 5. For women who it is recommended for, schedule a cervical cancer screening 6. For individuals age 45 or older, complete a colorectal cancer screening, pending coverage and provider direction 7. If they have pre-diabetes, get enrolled in a diabetes prevention program at CGMHSA 8. If they have diabetes enroll in a diabetes education program at CGMHSA,
Make contact with, on average, 20 community individuals weekly.
Appropriately navigate LHSA wellness participants to the next level of care as needed.
Disseminates health education and grant-related project materials.
Encourages preventive health behavior skills related to the new healthcare to health promotion program.
Will also be given the LHSA Mobile Market and Wellness monthly schedules to promote healthy eating at the grocery store on wheels that comes to these neighborhoods, as well as Prevention and Wellness services offered via the Mobile Wellness van.
Perform other duties as assigned.
Hourly Pay Range: $15.00 - $24.40
Minimum Requirements
High School diploma or GED and three (3) years of related experience required.
