Job Description & Requirements Specialty: Social Worker Discipline: Social Work Start Date: 04/20/2026 Duration: 13 weeks 36 hours per week Shift: 12 hours, days Employment Type: Travel Social Worker – Care Coordinator Location: 450 Northside Cherokee Boulevard, Canton, GA 30115 Duration: 13 Weeks Schedule: 3x12 Hour Shifts (36 hours/week) – Days, 8:00 AM – 8:30 PM (varies) Important Note: No local candidates within 50 miles. Job Summary The Care Coordinator Social Worker collaborates with patients (neonate through geriatric), families, medical staff, and community agencies to assess hospitalization needs, determine appropriate levels of care, and coordinate post-hospital services. This role provides psychosocial assessments, crisis intervention, and discharge planning support to ensure smooth care transitions and effective clinical resource management. Minimum Requirements Experience: Minimum 2 years of experience within the specialty Acute Care Hospital experience required Education: Master of Social Work (MSW) or Bachelor's in a related field with strong clinical background Licensure/Certification: LCSW Certification required BLS Certification required Preferred: Certified Case Manager (CCM) Key Responsibilities Care Coordination & Case Management Screen cases pre-admission, admission, concurrent, and post-discharge per department policies Evaluate appropriateness of hospitalization and level of care Collaborate with clinical teams to prioritize discharge planning needs Coordinate post-hospital care plans with healthcare providers and community resources Patient & Family Support Conduct comprehensive psychosocial assessments Provide counseling and crisis intervention for patients and families Educate patients and families on post-hospital care options Provide emotional support and care transition guidance Documentation & Compliance Document assessments, care plans, and progress in the medical record Provide clinical documentation for reimbursement and external reviews Issue Notice of Non-Coverage letters when required Maintain compliance with hospital policies and payer guidelines Interdisciplinary Collaboration Work with physicians, nursing staff, financial counselors, and internal departments Refer cases to departments such as: Risk Management Infection Control Pharmacy Respond to internal and external referrals Resource Coordination Maintain updated community resource and referral networks Connect patients with social services, financial assistance, and community programs Additional Duties Participate in utilization review Assist with appeals related to care coordination denials Provide education and in-service training to hospital staff Perform other duties assigned by leadership Patient Population Served The role requires competency in serving multiple patient populations: Neonate Child Adolescent Adult Geriatric Skills & Competencies Required Skills Strong interpersonal and communication skills Ability to work with patients, families, and interdisciplinary teams Knowledge of care coordination, social services, and healthcare systems Strong problem-solving and decision-making abilities Language & Communication Ability to present information clearly Ability to interpret technical documents and regulations Strong verbal and written communication skills Mathematical Skills Basic mathematical proficiency Reasoning Ability Ability to interpret written and verbal instructions Ability to solve practical problems in complex environments Service Excellence Expectations Provide clear information and education to patients and families Respond promptly to patient and staff needs Demonstrate professionalism, respect, and empathy Deliver emotional support and effective problem-solving Supervision Reports To: Director/Manager, Care Coordination / Social Services Supervisory Responsibilities: None Working Conditions Work may involve exposure to: Infectious diseases Hazardous materials Needlesticks Blood and body fluids Hepatitis B vaccination is available to employees.
