The program focuses on:Outreach and EngagementComprehensive Assessment & Care PlanningEnhanced Coordination of CareHealth PromotionTransitional Care ServicesMember and Family SupportCommunity Resource and Social Service ReferralsThe ultimate goal is to improve care coordination, address social determinants of health (SDOH), and reduce unnecessary healthcare utilization for high‑need Medi‑Cal beneficiaries.Job Overview Reporting to the Program Manager, the Case Manager delivers high‑quality, client‑centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact.Key ResponsibilitiesConduct initial and quarterly psychosocial assessmentsCollaborate with participants on individualized service plans and housing stability goalsBuild trusted, professional relationships with clientsProvide outreach, housing navigation, and ongoing supportFacilitate referrals to healthcare, mental health, substance use, and community servicesSupport clients in obtaining employment, benefits, and education opportunitiesAccompany clients to key appointments as neededUse a harm reduction, strength‑based, and client‑focused approachGrowth Opportunities We are deeply invested in staff development.
