Apart from comprehensive therapeutic support and assistance to anyone dealing with issues related to mental and behavioral health, we also facilitate medication evaluations and management in our care routine. This is in addition to therapeutic support services across the lifespan starting at ages 5 to 99. Full time applicants required. Applicant should be a licensed Social Worker associate, licensed Counselor associate, or licensed Psychologist associate with experiences in mental health and substance abuse issues across the lifespan. Provisional licensees are welcome to apply.
Social Workers will work directly with patients and their families. Social Workers should be patient, compassionate, calm and able to manage stress. They may need to provide treatment to patients in lieu of medical professionals in the event of emergencies while also providing therapeutic support to patients and their families as needed. Social Workers may also have the following duties:
Evaluate patients (existing, new and prospective) based on their needs, limitations and desires
Address patient concerns and goals while maintaining constant communication with the patient as well as a provider.
Build rapport with patients and their families.
Develop care plans for patients. Duties include but are not limited to -
Performing initial evaluations of client's needs and developing customized treatment plans.
Educating clients on stress, trauma, work related and other emotional/psychological coping mechanisms.
Providing individuals with the resources they need to reach personal goals.
Monitoring client progress and modifying their treatment plans as needed.
Coordinating with social and medical agencies, healthcare providers, and community outreach programs to provide clients with needed services.
Completing continuing education courses to maintain National Certification and stay up-to-date on the latest evidenced based practices.
Individual and group counseling.
