The ideal candidate will conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes. Key ResponsibilitiesConduct health assessments and focused physical examinations related to wound, ostomy, and continence careDevelop and implement individualized care plans based on patient needs and diagnosesAssess, monitor, and document patient response to wound care treatments and therapiesProvide care for complex wounds, fistulas, ostomies, and continence issuesRecommend appropriate treatments, medications, and interventionsEducate patients, families, and staff on wound care management and prevention strategiesParticipate in quality improvement initiatives, including incidence and prevalence studiesCollaborate with physicians and interdisciplinary teams for coordinated patient careAssist in developing standards of care and selecting appropriate supplies and equipmentIdentify patients at risk for pressure injuries and initiate preventative measuresEnsure compliance with data security, confidentiality, and patient rights policiesSupport staff training, orientation, and continuing education programsRequired QualificationsActive New York State RN LicenseBLS Certification (AHA)Minimum 2 years of RN experience in wound care and ostomy careCertifications (One Required):CWOCN / CWON / CWCN – WOCNCBCWS – American Board of Wound ManagementWCC – National Alliance of Wound Care and OstomyPlease submit your resume toor Call .
