Job DetailsDescriptionWhat We’re Looking For in a Clinical Team Leader:Compassionate and caring attitude with a genuine concern for patientsStrong leadership, problem-solving, and decision-making skillsExcellent interpersonal skills to connect with patients, families, and team membersPrevious supervisory experience required (minimum 2 years)In-depth understanding of hospice care and clinical protocolsKey ResponsibilitiesSupervise and coordinate the delivery of high-quality patient care on a 24-hour basisManage personnel actions such as hiring, onboarding, performance reviews, and schedulingCoordinate patient care issues with families, physicians, and healthcare providersEnsure complete, timely, and accurate documentation of patient care in compliance with policies and regulationsLead interdisciplinary group (IDG) meetings and case conferences to align care goalsOversee regulatory compliance, clinical quality improvement, and staff development initiativesReview and implement policies and procedures to maintain agency standardsCover Management Back-Up (MBU) responsibilities as assignedCase manage patients when needed or requestedRequirementsCurrent RN license in North Carolina2 years of supervisory experience required5 years of recent clinical experience in hospice, home health, or equivalentCurrent CPR certificationKnowledge of reimbursement sources, Medicare/Medicaid regulations, licensing laws, and accreditation standardsMust meet pre-employment and ongoing state and federal training requirementsWhat To Expect During The Hiring ProcessHR or the hiring manager will contact you within 1–3 business days after application submissionParticipate in 1–2 interviews (virtual or in-person)Review the job offer, complete new-hire paperwork, and attend orientationReceive clinical skills training and ongoing support to excel in your roleAbout The RoleAs a Clinical Team Leader, you’ll provide leadership to ensure compassionate and high-quality care for our patients.
