Licensed Practical Nurse (LPN) Home Health HH/LPN Visits/Day: 6-7 Years of experience REQ: 1 First-timers accepted: Yes Weekend REQ: every 4th weekend Certs REQ: BLS, IV Cert Skills REQ: Home Health, Phlebotomy, Wound Care/Wound Vac, Wound drains/JP/Hemovac, Vital Sign monitoring Is on-call REQ? Yes, 3-5 nights per month Will clinician float within scope to meet facility needs? Within service line Open to accommodating block schedule? No Locals accepted: No, home address must be at least 75 miles from facility Pending License accepted: Yes Territory/County covering: 40-80 miles. Mileage reimbursed? Mileage will be reimbursed @ $0.55/mile. RTO Restrictions: No more than 7 days Guaranteed Hours: 3 shifts per 13 weeks. The cancelled shifts must be the regular shift length to which the Contractor Personnel is assigned Special Requirements: Skilled Home Health experience required. Hospital based preferred. PREFERRED: MS Team: Antibiotic infusion, lab draws, IV care, ostomy care, disease management, assess socioeconomic and medical needs, education Cardiac Team: Infusion(inotropes, dobutamine, milronine), IVP Lasix; post-op PREFERRED: pluerx catheters, Port access (maintenance and care), PT/INR finger sticks, ventilator knowledgeable, compression wraps / layered compression Will be placed into an every 4th weekend rotation to work both Saturday and Sunday. Will be placed into the overnight on-call rotation for up to 5 nights/month (not scheduled to take call on a night that they were not scheduled to work during the day). We request at least 7 days' notice to terminate the contract before 13 weeks. We will require return of all BJC issued supplies and equipment upon the completion of her contract, including, but not limited to, laptop, cell phone, nursing bag, equipment and trunk supplies. Cardiac: CADD pumps Hand held ECG monitor & interpret telemetry wound vacs Must be proficient in detailed head to toe assessments, skin and wound care and assessments including measuring wounds, taking photos, and understanding wound products.
