Case Manager-Heart Grant

BASIC PURPOSE OF THE JOBThe Case Manager coordinates and delivers appropriate services to targeted program population.ESSENTIAL FUNCTIONSMay participate in initial and ongoing assessments of clients.Provide weekly/biweekly participant contact to facilitate program retention, completion and access to needed support services, including services related to employment and economic stability through one-on-one meetings.Using assessment will determine core and supplemental curriculum to be addressed in Case Plan.Training and familiarity with all curriculums and willingness to facilitate/co-facilitate classes when assigned.Develop strategies to ensure follow-up to participants on referral services outside of the program and provide sustained support.Utilizes client needs and motivational interviewing to determine referrals to needed resources.Chart goals, objectives, progress and milestones, including employment and education plans.Provide follow-up support services to assist participants with job placement, retention and advancement.Provide updates to referral sources, sharing pertinent information about client progress, critical trends, barriers and solutions to achieving outcomes; and relationships with other organizations/communities.Participates in monthly supervision.Ability to work evenings and Saturday s to meet with participants.Implement Saint Francis policies and follow directives as required. Follows and adheres to all pertinent Standard Operating Procedures (SOP's), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.Is knowledgeable of and follows all safety procedures.Reports unusual incidents through appropriate Risk Management, clinical and safety channels.Ensures clients' rights are protected.MINIMUM REQUIREMENTSBachelor's degree, prefer a degree in Social Work, Human Services or related field.Must be 21 years of ageMust pass all required organizational background and health screenings for this program.Must have ability to work from home, including reliable internet accessMust have a valid driver's license, acceptable driving record and reliable transportationLifting requirements of 50 lbsREQUIRED SKILLS, KNOWLEDGE, AND ABILITIESMandated reporterReceives training in trauma informed practice and incorporates knowledge in day-to-day practiceUnderstanding of family centered practice, genograms, ecomaps, and social historiesAbility to engage clients in working relationshipsDemonstrates effective written and verbal communication skillsAbility to travelConsistently exercises discretion and independent judgment in performance of duties in conformance with applicable policies, procedures, statutes, rules and regulationsAbility to work with minimal supervisionHandles multiple prioritiesIndependent discretion/decision making within the scope and responsibility of the positionManage emotions and can make decisions under pressureManage stress appropriatelyWorks alone effectivelyWorks with others effectively/teamworkAbility to work with diverse populations
