Clinical Care Manager

Job Title: Clinical - Care Manager (RN)Duration: 6 months contract on W2 (Possibility to extend or convert)Location: Remote – OH and In Person (Must reside in Northeast Ohio- Loraine, Medina, Lake, Cuyahoga areas.)Shift Type: 8-5 Mon-Fri ESTPay Rate: $40 to $46/hr. on W2NOTE : In field about 50-60% of the time – individual can determine the days in field and manage their own schedule, The rest of the time will be documentation from home  Reimbursed for mileageDay to Day Responsibilities :Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their familiesLog on laptop top by 8am, review emails, tasks, and voicemails and respond to any urgent needs.Review calendar for possible meetings and/or scheduled visits.Staff typically arrange their days with the expectation of 5-7 visits required weekly.Documentation must be completed within 24 hours.Requirements:2-4 years of experience nursing, case management, home health, reliable vehicle for weekly travelWorks well independently, troubleshootingComputer Literate IS A MUST (knowledge of Microsoft) Excel, Word, Team, Outlook, One note, One Drive, PowerPoint, Explorer, ChromePerformance expectations:5-7 in home visits weekly, ensuring documentation is completed within 24 hours.Meeting required turn-round times for processes, and completing required trainings timelyEducation/ Licenses:Degree from an Accredited School of Nursing and 2 – 4 years of related experienceOhio RN Licensure
