Primary Care Physician

To achieve this, we seek primary care physicians who desire purposeful work and personal opportunity to join the ChenMed family. Responsibilities Provide independent care for patients with acute and chronic illnesses in older adults. Takes full accountability for patient care and outcomes; seeks specialist consultation when needed while remaining involved in detailed care. Engages with hospitalists during inpatient stays and with transitional care teams for homebound, SNF, LTC, and other settings. Leads a care team that includes a care promoter, care facilitator, and care coordinator. Supports center management, covers for other providers, and assists in recruitment of patients and additional providers. Maintains documentation, health promotion, and care coordination for all patients. Participates in risk and quality management programs, clinical meetings, and related meetings that promote patient health and company goals. Adheres to departmental goals, standards of performance, regulatory compliance, quality patient care compliance, and organization policies and procedures. Metrics regularly measured for PCP performance include patient admissions per thousand, use of 18‑21 appointment slots per day, CGCAHPS scores, clinical gap closures, and medical cost measures. Each PCP works toward these targets with center and market leadership and their care teams. Qualifications Availability and accessibility: open for walk‑in visits and prompt response to calls and messages. Service orientation: provide care as you would want for a family member. Evidence‑based medicine: stay updated on clinical, nutritional, and lifestyle interventions. Physician leadership: continuously develop leadership skills for benefit of patients, teams, center, and company. Quality focus: work with teams to achieve high quality outcomes and improve practice operations. Influence: competently and compassionately influence patients, teams, and self to achieve best outcomes. Self‑care: maintain physical, mental, social, and spiritual wellness to provide best care. Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software. Willingness and availability to travel locally, nationally for initial training, and occasionally regionally for recruiting or training. Fluency in English; other language fluency required only if specified by the center. Use and exercise independent judgment in clinical decision‑making. Salary and Benefits Pay range: $221,141 – $315,915.
