Speech Language Pathologist (SLP)Location: ASVH-Flagstaff, 2100 N. Gemini Rd., Flagstaff, AZSchedule: Monday–Friday, 8:00 AM – 5:00 PM | No weekends | Typically 40 hours/week | No overtimePosition Summary:We are seeking a compassionate Speech Language Pathologist (SLP) to evaluate and treat residents/patients with speech, hearing, swallowing, and cognitive disabilities. The SLP will develop and implement treatment plans in compliance with state and federal guidelines and under a physician's direction.Essential Duties:Evaluate residents/patients and develop individualized care plansProvide direct therapy based on established treatment plansPlan and conduct rehabilitative programs to restore communication and/or swallowing abilitiesAdminister, score, and interpret speech and hearing testsInstruct residents/patients, families, and caregivers on therapy exercisesMaintain accurate and timely documentation per company policy and HIPAA guidelinesCollaborate with rehab therapists and nursing staff in a team environmentEnsure punctuality and regular attendanceOther Duties:Develop short- and long-term treatment goalsCommunicate therapy programs to families/guardiansMaintain written documentation including evaluations, daily notes, and weekly summariesPerform other duties as assignedQualifications & Skills:Bachelor of Science in Speech Language Pathology requiredActive SLP license in the State of ArizonaKnowledge of effective speech therapy techniques and geriatric careStrong clinical skills and collaborative mindsetEntry-level candidates are encouraged to applyRequired Documentation:Resume & Cover LetterLevel 1 Fingerprint Clearance CardOIG & APS ClearanceCPR/BLS CertificationLicense/Certification for positionVaccine StatusTB Skin TestHepatitis B vaccination or declinationAgency Skills Assessment PacketHow to Apply:Please submit all required documentation with your application to be considered.
