Care Manager (Supervisor of Direct Care Services)

Role SummaryThe Care Manager plays a key leadership role in delivering high-quality, client-centered care. This position is responsible for coordinating, implementing, and supervising care services while supporting caregivers and maintaining strong relationships with clients, families, and referral partners.Key ResponsibilitiesCare Planning & Client SupportDevelop, implement, and regularly update individualized Care Plans based on client needsConduct new client assessmentsPerform client and caregiver introductionsVisit clients in homes, hospitals, and rehabilitation facilitiesCaregiver Supervision & DevelopmentSupervise, coach, and support caregivers to ensure high-quality care deliveryProvide ongoing training, including infection control and emergency proceduresConduct announced and unannounced quality assurance visitsAddress performance concerns and administer discipline when necessaryAssist with caregiver onboarding and orientationCommunication & CoordinationServe as the primary point of contact for clients, families, caregivers, and referral partnersCoordinate care with external providers (e.g., home health, hospice, facilities)Collaborate with the Staffing Coordinator to ensure appropriate caregiver-client matchesFollow up on client and caregiver feedback, including surveysDocumentation & ComplianceMaintain accurate, timely documentation in agency systemsManage care notes for long-term care insurance providersTrack referrals and maintain CRM recordsOn-Call RequirementsServe in an on-call rotation to support clients and caregivers during evenings and weekendsRespond to urgent situations, including staffing gaps or client care issuesPerform caregiver and/or client support, including in-person visits, introductions, and new client assessmentsOperational SupportSupport scheduling needs as requiredParticipate in team meetings and ongoing training opportunitiesServe as a backup direct care provider when necessaryCross-training in other areas to assist with office operations, as neededWork as otherwise directedQualificationsRequired:2+ years of experience in a care-related roleSupervisory experienceValid drivers license and proof of insuranceAbility to travel locallyBasic computer skills (e.g., Microsoft Word, Excel, data entry)Ability to pass a criminal background checkSkills & Competencies:Strong interpersonal and communication skillsAbility to work effectively with co-workers, clients, caregivers, families, and referral partnersSound judgment and problem-solving abilitiesAbility to lead, coach, and hold team members accountableSchedule & Work EnvironmentFull-time, MondayFriday (8:30 AM 5:00 PM; some flexibility required)Participation in on-call rotation (evenings/weekends)Frequent local travel requiredCompensation details: 50000-50000 Yearly SalaryPIb44123cd94b4-26289-40281138
