The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve. Thome PACE (Program of All-Inclusive Care for the Elderly) provides a welcomed alternative to nursing home care for participants. The PACE model of care allows seniors aged 55+ to remain independent in their homes and communities while receiving the integrated social, wellness, and medical care services aging adults require.Learn more about Thome PACE at: https://mybrio.org/medicare-pace-program-michigan/Visit MyBrioCareers.org to learn more about the benefits of working with us.OverviewThe Registered Dietitian (RD) applies advanced dietitian knowledge and skills to provide personalized nutritional care plans for older adults.What You'll DoPerforms nutritional assessment and reassessmentsIdentifies need and implements nutrition servicesProvides nutrition education to participants and caregiversAttends meetings and takes part in participant care planningWorks collaboratively with other IDT membersDocuments changes in medical recordsAssists in monitoring mealsCoordinates 24-hour care deliveryWhat It TakesBachelor’s degree or master’s Degree from an ACEND accredited program and completed the Dietetic Internship.Passed the CDR exam and maintain Michigan Registered Dietitian (RD) status in good standing. Those who are eligible to sit for the CDR exam may be considered.Must be CPR certified or willing to obtain CPR within 90 days if offered employment.Master's Degree plus approved work experience preferred.Certified Diabetes Educator (CDE) status preferred.Minimum of one (1) year working with the frail and elderly.Why Brio Living ServicesMake an impact on the lives of others.
