RN – Home HealthDirect Hire | Full-Time
Registered Nurses in the Home Health setting provide compassionate, patient-centered care in patients' homes while supporting treatment, rehabilitation, and long-term health management. This role requires strong clinical judgment, leadership, and the ability to coordinate care with physicians, families, and interdisciplinary teams to ensure patients receive safe, effective care outside of a traditional hospital setting.
Position Overview:
Permanent, full-time role
Provide skilled nursing care to patients in their homes
Coordinate individualized care plans in collaboration with physicians and care teams
Assess patient and family health needs and potential environmental health risks
Deliver education and support to patients and caregivers regarding treatment and recovery
Key Responsibilities:
Provide, arrange, teach, and supervise nursing care for home health patients
Perform treatments, diagnostic procedures, and preventive care as ordered by the patient's physician
Evaluate patients' physical and environmental conditions and identify health risks or concerns
Develop and coordinate individualized home health care plans
Support patient recovery and rehabilitation through skilled nursing interventions
Provide leadership and collaboration within the care team while supporting departmental goals
Requirements:
Current Registered Nurse (RN) license in the state of Illinois (Illinois Department of Professional Regulation)
Minimum of 1 year of nursing experience preferred
Valid driver's license required for travel to patient homes
Strong clinical assessment and critical thinking skills
Ability to work independently while coordinating care with a multidisciplinary team
Job Knowledge & Skills:
Comprehensive knowledge of current nursing practice
Understanding of community health principles and home health care delivery
Ability to provide leadership as a professional nurse and collaborate effectively with care teams
Strong understanding of patient population needs when assessing and planning care
