Together, we make a difference in our community!Position SummaryThe Physical Therapist (PT) is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a home care environment. The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers. Community/home health experience is a plus.Demonstrates good verbal and written communication and organizationPossess and maintain valid CPR Must have obtained in-person with a practical skills evaluation as part of the final test.Access to reliable transportation.
