Growing together.As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team - based structure and spend the majority of their time in the community engaging directly with members. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and / or behavioral health conditions is important for this role.Working Schedule: Schedule is Monday through Friday standard business work hours 8am to 5pm Central Standard Time. Local travel up to 75% of the time and mileage is reimbursed at current government rate. This is a Field-Based Role with a Home-Based Office.If you reside in Kansas City, KS, you will have the flexibility to telecommute* as you take on some tough challenges.Primary ResponsibilitiesAssess, plan, and implement care strategies that are individualized by the member and directed toward the most appropriate, lease restrictive level of careUtilize both company and community-based resources to establish a safe and effective case management plan for membersCollaborate with patient, family, and healthcare providersIdentify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive servicesCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare teamDocument all member assessments, care plan and referrals providedParticipate in Interdisciplinary team meetings and Utilization Management rounds and provide information to assist with safe transitions of careUnderstand insurance products, benefits, coverage limitations, insurance, and governmental regulations as it applies to the health planUnderstand role and how it affects utilization management benchmarks and quality outcomesYou'll need to be flexible, adaptable and, above all, patient in all types of situationsEngage members either face to face or telephonicallyHelp member set person-centered SMART goals and develop a care plan to achieve those goals with regular follow up calls and ongoing documentation of progress towards goals metAdhere to detailed, specific documentation requirements in the member's health recordProactively engage the member to manage their own health and healthcare using Motivational Interviewing SkillsAs needed, help the member engage with mental health and substance use treatmentUtilize solid skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and ability to manage multiple deadlinesYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required QualificationsHigh School Diploma/GED1+ years of experience assisting others with healthcare and/or social determinants of health needsIntermediate level of computer proficiency including MS Word, Excel and OutlookIntermediate level of familiarity with the resources available in the communityAbility to travel up to visit Medicaid members in their homes and / or other settings, including community centers, hospitals, or providers' officesAbility to comply with UHG Telecommuting Policy and have a designated workspace inside the home with access to high-speed internet availability Access to reliable transportation and & valid US driver's licensePreferred QualificationsCommunity Health Worker (CHW) State course completionCase management experienceExperience with arranging community resourcesExperience with electronic chartingBackground in managing populations with complex medical or behavioral needsKnowledge and/or experience with behavioral health or substance use disordersBilingual/SpanishAll Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment.
