TITLE Job Coach, Individual Employment ServicesJob Type: Part TimeImmediate Supervisor: Director of Employment ServicesSupervision Exercised: NoneSUMMARYUnder the supervision of the Director of Employment Services, this position is responsible for carrying out objectives of evaluation and training and maintaining a high standard of programming. The Job Coach is responsible for coaching individual placements, ensuring quality, program participant driven placements and customer satisfaction.ESSENTIAL FUNCTIONSMonitors proper implementation and documentation of the Individual Service Plans for each program participantCoordinates the overall implementation of the program as it relates to the goals of each program participant in his/her ISP in conjunction with Employment Services ManagementEnsures that program components meet the assessed needs of the program participants in their ISP in conjunction with Employment Services ManagementAssists in compliance with Agency policies and CARF regulationsResponds promptly to emergency situationsUses crisis intervention techniques in emergency situations when program participants are in danger of injury to themselves or othersEnsures that program participant notes are completed in a timely mannerActs as a liaison between the employer, persons served and Momentum WORK, Inc.Attends scheduled meetings and in-service trainings as requiredHandles approved petty cash disbursement as it relates to program implementationComplies with all Momentum WORK Inc. policies and proceduresCompletes all required paperwork in a timely mannerOther related duties, responsibilities, and special projects on an as needed basisPHYSICAL REQUIREMENTS:Ability to stand, sit for long periods of timeAbility to go up and down stairsAbility to walk for long periods of timeAbility to push or pull wheelchairs when necessaryAbility to speak, read, hear, and understand technical written materialsThe employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people servedFull awareness of environmental stimuliAbility to implement Crisis Management, Safety & Pro Act techniques (training included)Note: ADA accommodations available.QUALIFICATIONSHigh school diploma or equivalentAt least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferredDemonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related informationExcellent verbal and written communication skillsAbility to complete tasks with numerous interruptionsCompassion and respect for persons with disabilitiesAbility to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitationsMay need to be available during any disasters, per disaster planMust have reliable phoneValid CA Driver's License: must have an acceptable driving record as determined by criteria established by the Agency's insurance carrier and by Agency policyValid certification in CPR/First Aid (Training included)BENEFITS:Travel StipendCell Phone StipendPaid Time- Off403(b) Retirement PlanFriends & Family Referral BonusDirect DepositAnniversary BonusOn-the-job Training ProvidedUp to $750 Sign-On Bonus
