Job DescriptionRegistered Nurse Hospice Field RN5x 8h (Mon-Fri)Job DescriptionsThis job is considered a leader in the provision of patient care utilizing the Nursing Process within the framework of the Nurse Practice Act, ANA Code for Nurses and Scope & Standards of Practice. The RN effectively delegates, directs and assists licensed and ancillary team members; assumes accountability for quality patient outcomes; exhibits sensitivity to cultural, ethnic and religious diversity in all interactions; maintains involvement in activities aimed toward the achievement of unit and the department's strategic goals and objectives and demonstrates professional responsibility and accountability for his/her own practice and supports the company's philosophy of nursing.EducationRequired - Registered Nurse DiplomaPreferred - Associate's or Bachelor's degree in NursingExperience Required - 2 Years of Related ExperienceCertificationsRequired -Current RN License in the state of practiceCurrent Basic Life Support (BLS) certification from the American Heart Association Preferred - Certification in Case Management (CCM) orHealth Care Management (CPHM)KSAs• Proficiency in using computers, software, and web-based applications.• Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.• Good organizational and time management skills.• Conflict resolution skills and ability to demonstrate good judgment and decision-making in performing tasksJob Duties• Manages various aspects of the planning process in anticipation of the patient's movement through the healthcare system.• Uses care management policies and procedures, including applying evidence-based criteria and levels of care and continuing appropriateness.• Manages the care of assigned patients through the healthcare system based on the patient's individual needs.• Prepares, executes, and reinforces post-discharge care plan.• Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions, and communication style.• Performs other related duties as required.The above statements describe the general nature and level of work only. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.The normal routine involves no exposure to blood, body fluid, or tissue, but exposure or potential for exposure may occur.The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases.Because the incumbent works in a healthcare setting, there may be an occupational risk of exposure to hazardous medications or hazardous waste in the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning, and/or disposal of contaminated waste.
