Job Location: a base location that is flexible within the State of Missouri. This position is telecommuting-eligible if candidate meets all telecommuting eligibilities and qualifications. Why you’ll love this position: We are seeking an individual who is committed to the mission of keeping Missourian’s safe and healthy by completing timely care planning for Missouri’s Medicaid-eligible aged and disabled population that need in-home services tailored to meet their needs to successfully remain in their homes and communities in the least restrictive setting. A person in this position will exhibit exceptional communication skills, have the ability to interview others to gather pertinent information, show strong documentation practices, consistently display exemplary customer service, and demonstrate collaboration amongst an interdisciplinary team. What you’ll do: Lead phone interviews to determine & develop person-centered care plans Utilize a nationally-credited assessment tool to ensure program integrity Provide objective & accurate documentation to an interdisciplinary team Represent the program with exemplary customer service & call center etiquette Assume accountability & responsibility over program operations & success All you need for success: Minimum Qualifications Bachelor’s degree (substitutions may be allowed). Strong interviewing and communication skills Keen to detail and documentation Exhibit disability & cross-generational awareness Prior experience with customer service, call centers, and working with the elderly or persons with disabilities is preferred. A Bachelor’s degree from an accredited college or university; OR A Registered Nurse (RN) who is licensed and in good standing in Missouri; OR A Licensed Practical Nurse who is licensed and in good standing in Missouri with 1 or more years of experience working as an LPN. One or more years of experience as Social Services Specialist; OR Four or more years of experience with the Division of Senior and Disability Services or an Area Agency on Aging.
