Job ResponsibilitiesJOB SUMMARY:The RN will support primarily chronic care and wellness patients of child-bearing years from contemplating pregnancy, pregnant, and postpartum through the mobile (about four + counties) and fixed (Abbeville) BLOOM clinics. Supports in-person care, telehealth care and remote patient monitoring. Leads RPM operations for the BLOOM Clinics and supports clinical research. Performs other duties as assigned.Job Duties25% - Essential - Nursing Support:Provides nursing support for the CRH nurse practitioner and BLOOM operations: Incumbent will provide nursing support for clinical care delivered by the CRH nurse practitioner. This includes a broad scope of RN activities that include patient education, patient workup, blood draws, surveys, patient history, and information necessary for clinical encounter notes. Also includes providing education and counseling of patients up to limits of RN license.25% - Essential - Remote Patient Monitoring Enrollment:Serves as the first point of contact between Clemson Rural Health and RPM enrollees. Performs outreach to enrollees to educate on benefits of RPM program and schedule enrollments appointments. Perform enrollment in person and virtually, including collecting medical history and social determinants of health information and accurately documenting in enrollee's electronic health record. Ensures enrollee understanding of program, enrollee requirements, expected outcomes and enrollee responsibility. Obtains consent for enrollment and ensures upload to electronic health record. Provides RPM devices and educates patient on use. Excellent verbal and written communication skills.25% - Essential - Remote Patient Monitoring Management:Establishes care plans with patient and provider, along with realistic and achievable goals for enrollees. Ensures patients are meeting eligibility for RPM requirements. Reviews enrollee data data and communicates with enrollees and providers regarding readings outside of normal parameters. Manages enrollees per standing orders, including notification to providers of changes in enrollee status. Works across multiple platforms to ensure accurate documentation. Performs persistent follow up with enrollees who are not meeting program requirements and navigates/troubleshoots enrollee issues with meeting program requirements.15% - Essential - Medical Supply Inventory Management:Maintains inventory of RPM devices provided to patients. Ensures inventory availability and orders additional supplies when needed. Pre-registers devices in advance of mobile events for delivery to enrollees. Assists with prior authorizations and insurance requirements for replenishment of medical supplies and devices.10% - Essential - Sponsored Programs Support:Assists clinic staff and principle investigators in implementation of approved research protocols including clinical research. Includes successful completion of training required in the protection of human subjects research.Minimum And Additional RequirementsEducation - Graduation from an accredited school of nursing and related nursing experienceClinical rotation of some experience with patient care coordination and/or RPMMust have current unrestricted RN licensure; currently CPR certifiedWork Exp 1+ yearsPreferred QualificationsEducation - 1st Master's Degree in NursingWork Exp 2-3 years' experience in clinical setting; emphasis on care coordination and/or remote patient monitoring and telehealth; rural health clinic experience a plus.
