In this role, you'll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.Core Responsibilities:Provide in-person medical care to patients in designated post-acute facilitiesPerform assessments, manage treatment plans, and maintain accurate documentation in the EMRCollaborate with facility teams to support quality outcomes and smooth transitions of careCollaborate as applicable in Trumen's value-based care and transitional care programsDevelop strong working relationships with facility and clinical staffServe as a liaison between Trumen and facility teamsMaintain an on-site presence consistent with your agreed schedule and facility needsQualifications:MD or DO with a current, unrestricted Texas medical licenseBoard Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine1+ year of experience in post-acute, geriatric, or SNF care preferredComfortable practicing independently in skilled nursing facilitiesStrong clinical judgment, communication skills, and a patient-centered mindsetReliable transportation within the Brady areaAbout Trumen Physicians + Associates:Founded in 2010, Trumen is a physician-led, provider-centric organization focused on improving outcomes for patients in post-acute and transitional care settings.
