Your Role
Provide comprehensive primary care to older adult patients
Maintain full accountability for clinical outcomes and patient experience
Serve as a health coach, guiding lifestyle and long-term health improvement
Coordinate specialty and hospital care while staying actively involved
Lead and collaborate with an interdisciplinary care team
Supervise and remain accountable for Advanced Practice Providers (APPs)
Partner with medical and operational leadership to meet quality and cost goals

Qualifications
MD or DO required
Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
Active or eligible state medical license
