Summary:
The Hospice PCM / ADON will be responsible for implementing company policy and mission by maintaining appropriate staffing levels, meeting set standards and goals to support our Hospice DCS / DON in the effective running of hospice operations and delivering outstanding patient care, while taking care of & supporting our staff / care teams
Responsibilities:
Supervision, orientation, mentoring, developing and on-going evaluation of all patient care staff who are engaged in the delivery of care.
Assures compliance with agency policies and procedures, state and federal laws, regulations and accreditation standards.
Participation in the Manager on-call rotation
Assist with talent acquisition to ensure adequate staffing is maintained
Coordinate and facilitate IDT meetings
Assist with quality assurance and performance improvement efforts
Assist with patient case load, as needed
Manage IDT process for multiple Physicians and Teams
Experience:
Management, 1 year (Required)
Hospice Care, 2 years (Required)
Associates Degree in Nursing; Bachelors preferred
Valid RN Texas License
CHPN preferred
Valid Texas Driver's License and Proof of Auto Insurance
