Serves as co-leader in interdisciplinary approach to patient groups.Job Requirements:Education and Work Experience:Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016Licenses/Certifications:Physical Therapy licensure in all applicable states of practice: RequiredCardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: RequiredValid Driver's License (DL) and must be at least 21 yrs of age or older: RequiredFacility Specific License/Certifications:Basic Life Support (BLS) or Basic Life Support (BLS) or RQIBLS or Heartsaver CPR (CPR): RequiredPhysical Therapy licensure in state of practice: RequiredEssential Functions:Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.Provides therapy interventions using appropriate treatment protocols and techniques.
