Physical Therapist - Travel Contract

Active Indiana Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Experience performing comprehensive PT evaluations, developing and implementing treatment plans, and documenting per hospital policies and regulatory guidelines. Experience collaborating with interdisciplinary teams and providing patient and family education, including home exercise programs and discharge planning. Experience with safe patient handling and materials handling protocols in an acute care or hospital environment. Ability to complete timely medical record documentation including daily notes, goal writing, functional quality indicators, letters of medical necessity, discharge summaries, and charge entry. Maintain professional licensure in good standing and complete state mandated continuing education requirements. Preferred: Experience supervising or mentoring students or less-experienced clinicians.
