And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay.The Documentation Specialist will organize, communicate, process, and document information related to hospice certifications and consents according to state and federal regulations. Additionally, he/she will process and issue all patient admission and patient death letters.Work Location: 4266 Sunbeam Road, Jacksonville, FL. No work from home at this time.Days/Hours: Monday - Friday 8:00am - 5:00pm; 40 hours a weekCompensation: $20.00/hour base salary; plus full benefits package including 23 days annual PTOPosition DetailsObtain attending/referring MD and CHPC MD signatures on hospice certifications within two (2) days of admissionThis may require travel to the physicians' office and/or facility to obtain the signatureDocument signatures in SuncoastUpdate patient's chart with signed hospice certificationEnsuring consents are accurately signed and documentedProcess and issue all Admission and Death LettersProvides excellent customer service and maintains an amicable and professional working relationship with Community Hospice's referral sources and partners.Maintains a high level of confidentiality of all patient data as outlined in the Health Insurance Portability and Accountability Act (HIPAA).Meets the standards of the job, such as productivity, accuracy, quality standards, adherence to scheduleAbility to work overtime as neededPerforms all other duties as assignedEducation & ExperienceMinimum requirement of a high school diploma; Medical degree certificate, example CNA, Medical Coding, etc...or AA degree or higher preferred.5+ years customer service experience5+ years in a medical office environmentDesired Candidate KnowledgeIntermediate computer skills, intermediate typing and data entryExcellent written and verbal communication skillsExcellent time management and organizational skillsDemonstrates effective people skills and sensitivities when dealing with othersCommunity Hospice & Palliative Care, including all of its subsidiaries and affiliated companies, is committed to complying with all applicable provisions of the Americans with Disabilities Act (ADA), as amended.
