Find out what it means to Dare to Care Differently!Schedule:Monday - Wednesday - Friday (3 day shifts) 35 hour weeklyDay shift onlyNo weekendsFixed schedule/no on-callKey Responsibilities:Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;Develops and maintains a plan of care that describes interventions to attain expected outcomes;Implements and records interventions to attain expected outcomes in EMR;Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);Maintains a positive and respectful attitude to all patients and colleagues;Occasional travel to other nearby locations; andMedication administration and documentation.Requirements:Valid RN licenseBS/AA/diploma in nursing6 months of RN experience strongly preferred3 months of hemodialysis experience strongly preferredBLS certification (American Red Cross or American Heart Association Required); andAbility to lift up to 35 pounds is requiredThis is a safety sensitive positionBenefits:Depending on your job status, you may be eligible for the following benefits:Medical, Dental, Vision, & Life Insurance401(k) packageFlexible Spending AccountsEducation AssistanceCertification & Professional Membership ReimbursementCompensation:
