Job OverviewWe are seeking a casual physical therapist to join our outpatient rehabilitation flexible workforce (FWF) team!The physical therapist will provide PT services to adolescent through adult patients and the FWF team provides staffing coverage for multiple outpatient multi-specialty therapy clinics in the greater Twin Cities metro area. Preferred candidate will have experience in providing interventions for vestibular and concussion related impairments. Where training and experience is limited, Fairview offers mentoring and development opportunities for professional growth.In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.Casual position authorized to work up to 40 hours per week.Position provides coverage at various locations across Twin Cities metro including, Blaine, Burnsville, Eden Prairie, Edina, Eagan, Maple Grove, Maplewood, Minneapolis, St. Paul, and Woodbury.Continuing education opportunities.Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.As an academic health system with residency and fellowship programs and a rehab-focused clinical quality team, we have a collaborative culture that is centered on learning with an emphasis on evidence based, patient-centered care. As an industry leader in rehabilitative care with nationally recognized programs, our therapists play a critical role in helping patients reach their goals and live more fulfilling lives.ResponsibilitiesThe physical therapist partners with the patient and family while performing the Evaluation/Re-evaluationGathers pertinent dataPerforms examinationDetermines treatment diagnosisDevelops an individualized plan of care through collaboration with the patient and familyAnalyzes data to identify needs and potential risksDetermines prognosis for recoveryEstablishes individualized goals with patient and/or caregiverIndividualizes plan of care considering developmental level and cultural differencesImplements and modifies plan of care based on reassessment and patient responseCoordinates care and communicates effectively with interdisciplinary team for all aspects of patient careEstablishes appropriate discharge planPartners with patients and families to provide skilled interventionImplements plan of careAlters treatment/plan of care to reflect change in patient status and response to treatmentAssesses patient and/or caregiver learning needs and provides appropriate educationRequired Qualifications* Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota* Basic Life Support (American Heart Association or Red Cross)Preferred QualificationsDoctorate of Physical TherapyExperience with vestibular and concussion1 year experienceBenefit OverviewAlthough this is considered a non-benefit eligible position, Fairview offers employees the option to participate in the Fairview 403(b) plan, earn Sick and Safe Time, and early access to earned wages.Compensation DisclaimerAn individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions.
