RN Case Manager
An RN Case Manager is a registered nurse who coordinates and oversees patient care to ensure quality outcomes, cost-effective treatment, and seamless transitions across healthcare settings. They assess patient needs, develop and implement individualized care plans, collaborate with physicians and other healthcare professionals, and monitor progress toward recovery goals. RN Case Managers also educate patients and families, arrange follow-up services, and help navigate insurance and community resources. Typically licensed as a registered nurse and experienced in clinical care, they play a key role in improving patient outcomes and optimizing care delivery in hospitals, clinics, and community health programs.
