Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system.Make a DifferenceAs a Post-Acute Care Coordinator LPN on the IHCI Team, you will provide r emote patient monitoring supporting patients enrolled in a remote monitoring program by reviewing transmitted health data, providing patient outreach, and escalating concerns according to established clinical protocols. This role works closely with nursing leadership and care teams to promote early identification of health changes, improve chronic disease management, and enhance patient engagement-all in a remote, technology-enabled care model. Key Responsibilities include:Review and monitor patient-generated health data (e.g., blood pressure, weight, blood glucose, pulse oximetry) transmitted through a remote monitoring platform. Review and respond to care path focused questionnaires submitted by patients.Identify abnormal trends or readings and follow established clinical workflows for patient outreach and escalationOutreach to possible Remote Patient Monitoring patients and obtain consent.Conduct outbound phone calls or secure messages to patients to assess symptoms, reinforce education, and support adherence to care plansDocument patient interactions, clinical assessments, and interventions accurately in the electronic health recordCommunicate effectively with providers and their triage team, pharmacy, and other care team members to coordinate follow-up care as neededEducating patients on proper device usage, data transmission, and self-management strategiesReinforcement of care path zones based on vital sign trendsSupport quality, compliance, and safety initiatives related to remote patient monitoring servicesMaintain patient confidentiality and comply with HIPAA and organizational policies Exceptional Skills and QualificationsGraduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience required.Licensed as a Practical Nurse by the Indiana Professional Licensing Agency required.Previous experience in ambulatory care, chronic disease management, telehealth, or care coordination preferredStrong communication, critical thinking, and patient engagement skillsComfortable using clinical technology, electronic health records, and remote monitoring platformsAbility to work independently in a remote or hybrid environment while collaborating with a virtual care team
