They operate modern clinics where teams work closely with patients to improve health outcomes and provide a more supportive, relationship-based care experience.Job DescriptionThe Community Health Worker will serve as a key member of the clinic care team, supporting a dedicated patient panel in a fast-paced primary care environment.
